Care Plan for Chronic Myeloid Leukemia
Category: Hematology
Assessment
Patient presents with fatigue, weight loss, and splenomegaly.
Diagnosis
Risk for infection related to immunosuppression from leukemia.
Goals
Patient will remain free from infection as evidenced by normal white blood cell counts and absence of fever. Patient will verbalize understanding of the importance of infection prevention measures and regular monitoring.
Interventions
Administer prescribed tyrosine kinase inhibitors and monitor for side effects. Monitor complete blood count (CBC) regularly to assess for signs of infection. Educate patient on the importance of infection prevention measures such as hand hygiene and avoiding contact with sick individuals. Encourage regular follow-up appointments with a hematologist.
Rationale
These interventions aim to manage symptoms, prevent complications, and ensure the patient is well-informed about their condition.
Evaluation
Patient's infection status and white blood cell counts will be monitored to evaluate the effectiveness of interventions.
