Diagnosis

Risk for injury related to seizure activity and ⬆️hypertension .

Goals

Patient will be free from seizures and maintain blood pressure within normal limits. Patient will verbalize understanding of the importance of monitoring blood pressure and recognizing signs of eclampsia.

Interventions

Administer prescribed antihypertensives and anticonvulsants. Monitor blood pressure, urine protein levels, and signs of seizure activity. Educate patient on the importance of regular prenatal care and monitoring blood pressure at home. Encourage patient to maintain a low-sodium diet and avoid triggers that may elevate blood pressure.

These interventions help manage blood pressure, prevent seizures, and promote maternal and fetal health.

Evaluation

Patient's blood pressure, seizure activity, and understanding of eclampsia management will be monitored to evaluate the effectiveness of interventions.
