Diagnosis
Risk for impaired tissue perfusion related to inflammation and clot formation.
Goals
Patient will demonstrate reduced pain and swelling as evidenced by decreased erythema and tenderness along the affected vein. Patient will verbalize understanding of thrombophlebitis management strategies.
Interventions
Monitor affected area for changes in swelling, pain, and redness. Administer prescribed anticoagulants to prevent clot extension. Educate patient on the importance of leg elevation and use of compression stockings. Encourage early ambulation to promote circulation.
Rationale
These interventions aim to reduce inflammation, prevent clot extension, and promote venous return.
Evaluation
Pain, swelling, and erythema will be monitored to assess the effectiveness of interventions.
