Care Plan for Renal Calculi (Kidney Stones) Management

Assessment

Patient presents with severe flank pain, hematuria, and nausea.

Diagnosis

Acute pain related to renal calculi.

Goals

Patient will report pain relief at an acceptable level as defined by the patient. Patient will demonstrate understanding of strategies to prevent recurrence of kidney stones.

Interventions

Administer prescribed analgesics and monitor for effectiveness. Encourage patient to drink plenty of fluids to help pass the stone. Educate patient on dietary modifications to prevent recurrence, such as reducing intake of oxalate-rich foods. Monitor for signs of complications such as urinary obstruction or infection.

Rationale

These interventions aim to manage pain, facilitate stone passage, and prevent recurrence of kidney stones.

Evaluation

Patient’s pain levels and adherence to preventive strategies will be monitored during follow-up visits.
