Diagnosis

Risk for bleeding related to ruptured esophageal varices.

Goals

Patient will maintain hemodynamic stability and be free from signs of active bleeding. Patient will verbalize understanding of dietary and lifestyle modifications to prevent variceal bleeding.

Interventions

Monitor vital signs and assess for signs of bleeding and shock. Administer prescribed medications to reduce portal ⬆️hypertension (e.g., beta-blockers). Educate patient on avoiding alcohol and foods that may irritate the esophagus. Prepare patient for endoscopic treatment if necessary.

Rationale

These interventions help manage symptoms, prevent complications, and support patient understanding of esophageal varices management.

Evaluation

Patient's hemodynamic stability, signs of bleeding, and understanding of prevention strategies will be monitored to evaluate the effectiveness of interventions.
