Assessment

Patient presents with dry mucous membranes, decreased skin turgor, and dark urine.

Diagnosis

Risk for fluid volume deficit related to inadequate fluid intake.

Goals

Patient will maintain adequate hydration and normal fluid balance. Patient will demonstrate understanding of the importance of fluid intake.

Interventions

Monitor fluid intake and output, and daily weight. Encourage patient to drink fluids regularly and provide oral rehydration solutions if needed. Educate patient on the signs and symptoms of dehydration and the importance of maintaining hydration. Administer intravenous fluids as prescribed if oral intake is insufficient.

Rationale

Monitoring and encouraging fluid intake help maintain hydration. Education ensures patient understands how to prevent dehydration.

Evaluation

Assess for improvement in hydration status and fluid balance. Evaluate patient's understanding of hydration importance.
