Care Plan for Deep Vein Thrombosis (DVT)
Category: Cardiovascular
Assessment
Patient presents with swelling, pain, and redness in the affected leg.
Diagnosis
Risk for ineffective tissue perfusion related to venous thrombus formation.
Goals
Patient will maintain adequate tissue perfusion as evidenced by normal skin color and temperature in the affected limb. Patient will verbalize understanding of DVT prevention measures.
Interventions
Monitor for signs of DVT and assess limb regularly. Administer prescribed anticoagulants and monitor for signs of bleeding. Educate patient on the importance of leg elevation and avoiding prolonged immobility. Encourage patient to perform regular leg exercises and wear compression stockings.
Rationale
These interventions help prevent thrombus extension, promote venous return, and reduce the risk of complications associated with DVT.
Evaluation
Patient's limb perfusion and understanding of DVT prevention will be monitored to evaluate the effectiveness of interventions.
