Diagnosis
Risk for impaired skin integrity related to allergic reaction or irritant exposure.
Goals
Patient will experience relief from itching and inflammation. Patient will avoid known irritants and allergens.
Interventions
Identify and avoid exposure to known allergens or irritants. Apply topical corticosteroids and antihistamines as prescribed. Educate patient on proper skin care and use of hypoallergenic products. Monitor for signs of secondary infection and provide appropriate care.
Rationale
Avoidance of irritants and appropriate treatment reduce symptoms and prevent complications.
Evaluation
Assess for reduction in symptoms and improvement in skin condition. Evaluate patient's understanding of prevention strategies.
