Interventions

Monitor skin integrity and provide appropriate skin care to prevent breakdown. Educate patient on bladder training techniques and pelvic floor exercises. Advise patient to avoid bladder irritants such as caffeine and alcohol. Encourage regular follow-up appointments with a urologist.

Rationale

These interventions aim to manage symptoms, prevent complications, and educate the patient on managing their condition.

Evaluation

Patient's skin integrity and symptomatology will be monitored to evaluate the effectiveness of interventions.
