Assessment
Patient presents with chronic heartburn and difficulty swallowing.
Diagnosis
Risk for esophageal cancer related to chronic gastroesophageal reflux disease (GERD).
Goals
Patient will report a reduction in heartburn symptoms as evidenced by fewer episodes of reflux. Patient will verbalize understanding of the importance of lifestyle modifications and medication adherence.
Interventions
Administer prescribed proton pump inhibitors to reduce stomach acid. Educate patient on dietary modifications to avoid trigger foods. Encourage patient to maintain an upright position for at least 30 minutes after eating. Advise patient to avoid smoking and alcohol, which can exacerbate symptoms.
Rationale
These interventions aim to manage symptoms, prevent complications, and educate the patient on managing their condition.
Evaluation
Patient's symptomatology and adherence to lifestyle modifications will be monitored to evaluate the effectiveness of interventions.
