Goals

Interventions
Educate patient on the importance of vaccinations such as pneumococcal, meningococcal, and Haemophilus influenzae type B. Advise patient on infection prevention measures, including good hand hygiene and avoiding contact with sick individuals. Monitor for signs and symptoms of infection and provide prompt medical attention if needed.
Rationale

Evaluation
Patient's infection status and adherence to vaccination schedules will be monitored to evaluate the effectiveness of interventions.
