Care Plan for Aortic Dissection Management

Category: Cardiovascular

Assessment

Patient presents with severe chest pain radiating to the back, hypotension, and a history of ⬆️hypertension .

Diagnosis

Risk for decreased cardiac output related to aortic dissection.

Goals

Patient will demonstrate stable hemodynamic status as evidenced by normal blood pressure and absence of chest pain.

Interventions

Monitor vital signs and hemodynamic status closely. Administer prescribed medications to manage blood pressure and reduce stress on the aorta (e.g., beta-blockers). Prepare patient for possible surgical intervention if indicated. Educate patient and family on the importance of medication adherence and recognizing signs of aortic dissection.

Rationale

These interventions aim to stabilize hemodynamic status, manage symptoms, and prevent complications associated with aortic dissection.

Evaluation

Hemodynamic status and symptom resolution will be monitored to assess the effectiveness of interventions.
