Diagnosis
Impaired physical mobility related to decreased muscle strength and coordination secondary to stroke.
Goals
Patient will demonstrate improved mobility and ability to perform activities of daily living (ADLs) with minimal assistance.
Interventions
Encourage regular physical and occupational therapy to improve mobility and coordination. Educate patient and caregiver on the use of assistive devices and adaptive equipment. Monitor for signs of complications such as aspiration pneumonia or deep vein thrombosis. Provide emotional support and refer to support groups and counseling as needed.
