Assessment

Patient presents with complaints of excessive daytime sleepiness, loud snoring, and episodes of breathing cessation during sleep.

Diagnosis

Sleep pattern disturbance related to obstructive sleep apnea.

Goals

Patient will experience improved sleep quality as evidenced by reduced daytime sleepiness and decreased apneic episodes. Patient will demonstrate correct use of CPAP (Continuous Positive Airway Pressure) equipment.

Interventions

Educate patient on the use and maintenance of CPAP equipment. Encourage weight loss if patient is overweight or obese. Advise patient to avoid alcohol and sedatives before bedtime. Refer patient to a sleep specialist for further evaluation and management.

Rationale

These interventions aim to improve airway patency during sleep and enhance overall sleep quality.

Evaluation

Patient’s sleep quality and adherence to CPAP therapy will be monitored during follow-up visits.
