Patient presents with decreased ability to perform activities of daily living (ADLs) due to a recent stroke.

Diagnosis

Self-care deficit related to impaired physical mobility and cognitive function.

Goals

Patient will demonstrate improved ability to perform ADLs with or without assistive devices. Patient will participate in occupational therapy sessions as scheduled.

Interventions

Refer patient to an occupational therapist for evaluation and treatment. Educate patient and family on the use of assistive devices to enhance independence. Encourage participation in daily activities to the extent possible, gradually increasing complexity. Provide support and encouragement to boost patient’s confidence and motivation.

Rationale

These interventions aim to enhance the patient’s functional ability and promote independence in ADLs.

Evaluation

Patient's progress in occupational therapy and ability to perform ADLs will be assessed during follow-up visits.
