Risk for decreased cardiac output related to increased systemic vascular resistance.

Goals

Patient's blood pressure will be maintained within normal limits (less than 140/90 mmHg). Patient will verbalize understanding of the importance of lifestyle modifications in managing ⬆️hypertension .

Interventions

Monitor blood pressure regularly and document readings. Educate patient on dietary changes such as reducing sodium intake and increasing consumption of fruits and vegetables. Encourage regular physical activity, aiming for at least 30 minutes of moderate exercise most days of the week. Discuss the importance of medication adherence and potential side effects of prescribed antihypertensives.
