Care Plan for Gout
Category: Metabolic Disorders
Assessment
Patient presents with severe joint pain, swelling, and redness, typically in the big toe.
Diagnosis
Acute pain related to uric acid crystal deposition in the joints.
Goals
Patient will report decreased pain levels and maintain joint function. Patient will verbalize understanding of dietary and medication regimen to prevent gout attacks.
Interventions
Administer prescribed nonsteroidal anti-inflammatory drugs (NSAIDs) and colchicine. Educate patient on dietary modifications to reduce uric acid levels, such as avoiding purine-rich foods (e.g., red meat, shellfish) and alcohol. Encourage increased fluid intake to promote uric acid excretion. Monitor joint function and assess for signs of tophi formation.
Rationale
These interventions help manage symptoms, prevent complications, and promote overall well-being in patients with gout.
Evaluation
Patient's pain levels, joint function, and understanding of management strategies will be monitored to evaluate the effectiveness of interventions.
